RA REQUEST
A
Return Authorization (RA) form is required for all MAG Maintenance
Technologies and/or MAG Fadal components and part returns to
MAG
MT.
This form is not to be used to return any Machine or Rotary Table to
MAG.
Instructions:
1.
Use ONE RA form for each Invoice. Multiple parts
with the same part number may be identified by the
"Quantity" box below, if the symptoms/condition of the
components are identical. For example, if there are two different
part numbers being returned from the same Service Call, then two
RA's are to be used. If there are three parts with the same part
number from the same Machine or Rotary Table and from the same
Service Call, use one RA form and identify "3" in the
quantity box. If there are two components with the same part
number, but from two different machines, then use two RA forms,
one for each component. If there are two Service Calls on
different days for the same component on the same machine, then
use two RA forms, one for each component, representing two Service
Calls.
2.
Fill out the heading for each RA returned, including the name of
the person filling out the form, and the name of the Service
Technician who replaced the component. Mark the check box to
confirm that you accept and understand
MAG
MT
’s Return policy.
3.
Select ONE Reason for Return column from those listed
and provide the information required in each box in the column. If
two components are returned with the same part number from the
same machine, but the Reason for Return is different for each,
then use two RA forms.
A.
Machine or Rotary Table Warranty:
Component or part is covered by the
Machining
Center
or Rotary Table warranty for the period beginning at date of
installation noted on the returned Warranty Registration Form.
WRF must be returned within 15 from the date the
installation is complete.
B.
Part Warranty:
Components or parts that are replaced on out-of-warranty
Machining
Centers
or Rotary Tables are warranted for a period of 12 months from the
date of shipment.

invoice number for the failed part and an attached copy of the
Service Order to the customer are required to complete the credit.
Credit will be issued at the invoiced price.
C.
Core Return (Failed Part):
When a Core charge has been invoiced for a New or Remanufactured
component, the failed like component may be returned for refund.
Credit will be issued at stated Core Charge pricing.
D.
Return for Restock:
Components returned for restock must be less than 1 year old, in
original packaging, and in resellable condition. Credit will be
discounted 15% for Restocking Charge as long as the original order
included the machine serial number.
Credit will be discounted by 50% if no machine serial
number was included on the original order. Full Restocking credits
will be allowed up to 60 days after a component is discontinued.
4.
A valid Machine or Rotary Table serial number must be included on
all RA's. Exceptions would be Return for Restock, parts that were
never installed on a machine, and freight damage. If any
Replacement Part is installed onto a machine (even temporarily),
and returned for any failure reason, please include the model and
serial number.
5.
Components and parts returned for warranty credit must be returned
within 30 days of the Service Call date. Parts returned at later
dates will be returned to the sender.
6.
Each component part returned must be packaged together with its RA
form. Protect the form from oily components by using a plastic
ziplock bag. Do not package multiple components within one box
without attaching RA forms to each respective part. Do not ship or
mail RA form separately from component, otherwise, the part will
be returned to sender at customer expense.
7.
A copy of the Service Order as presented to the customer and

invoice (if available) for the part is required for "Machine
Warranty" or "Part Warranty" returns.
8.
Fill out the "Reason for Return" box with a descriptive
but brief reason. Avoid uninformative descriptions, such as
"Bad" or "Don't work".
9.
Provide component serial numbers and/or date codes when available.
10.
Do not create permanent markings on our products, doing so will
result in no credit and void warranty. Do not dis-assemble
assemblies or take apart components. Do not return incomplete kits
or partial kits.
11.
Warranty Parts returned are subject to validation testing and may,
at
MAG
MT
’s discretion, be returned to sender if no problem is found (NPF).
We reserve the right to reverse any credits based upon the result
of the testing.
12.
Mark on the outside of every package with "RA#" and
5-digit authorized number on all returns.
13.
After obtaining RA Number,
MAG
MT
needs to receive the shipment within 15 days. Otherwise, the RA
will be deleted.

REPLACEMENT PARTS
WARRANTY
MAG
warrants its NEW and REMANUFACTURED parts for a period
of 12 months from the date of shipment.
Part warranty on New Machines is for 1 year after machine
installation date.
Part warranty on Remanufactured Machines is for 1 year after machine
installation date.
If a replacement part is installed by a NON-Authorized
MAG Service Technician, the warranty shall be void.
